Illness & Prevention Program: Pacific Western Distributors Association

A written Illness & Injury Prevention Program is required for all employers by California State Law, SB198. Suhr Risk Services model assists our customers with a template that can easily be adapted to their organization. It is recommended that the document include the specific safety practices that you have developed.

Other considerations should include:

  • Use of occupational injury clinics designated by the employer.
  • Pre-designated physician forms required by LC3550 and AB749
  • CAL/OSHA 300 log
  • Company policy on "First Aid" claim payment
  • Required workers comp claims reporting forms (DWC-1, 5020)
  • Claims reporting checklist
